- Member or Members appointed representative authorizes SPRX, Inc DBA InjuryScripts Pharmacy to refill prescription(s) based on days supply or specific request by the patient or provider.
- Member acknowledges they have experience using Mail order or other automatic delivery programs at the Pharmacy.
- Prescription(s) are not eligible for reimbursement from federal or state programs, including, but not limited to Medicare, Medicaid and Tricare. Member agrees not to submit for or request reimbursement from federal or state programs for these medications.
